Policy

We strive to provide our patients with upmost respect and take pride in individualizing each patient's treatment to transformation. We would like to take this opportunity to welcome you to our office and at the same time answer some of the questions you may have about our office.

Most patients tend to reach a comfort level with one specific provider/technician and we will make our best effort to accommodate your request and your availability whenever possible. However, there will be times when their schedules are full or they are out of the office and you will be offered an appointment with a different provider/technician.

Exclusion/ Behavior: Rude, inappropriate behavior of any sort will not be tolerated. Sexual Comments or remarks will not be tolerated and will result in immediate termination of the session. We have the right to refuse or terminate a service at any time for any of the above reasons. Payment for the entire service will be expected. We are professionals  and expect to be treated professionally.

Appointments:

Patients are seen by Appointment Only. After scheduling your appointment, our office will email your patient packet and/or consents from our scheduling system Aesthetics Pro. We ask that you complete your paperwork online prior to your appointment to allow us to dedicate all our time and attention to you during your scheduled time.

If you are not able to complete your paperwork online, please arrive 15 minutes prior to your appointment to complete your paperwork. Scheduling is based on a first come first served basis. 

Late Arriving: If you are running late to an appointment we ask that you would please call the office to notify us. We may have other patients scheduled after you and it may result in a shortened service or our inability to take care of you at that time. If you arrive more than 10 minutes late we will reschedule your appointment.

Appointment Reminders: A call/email/text will remind you of your upcoming appointment 48 hours in advance. 

Cancellations:

These fees pertain to ALL Appointments held with Reveal Medical Aesthetics, Inc.

Cancelling an appointment with no charge: To cancel an appointment with no charge we require a twenty-four (24) hour notice. This notice must be given directly to Reveal Medical Aesthetics, Inc. Notice provided to your practitioner/technician is not sufficient.

Cancellation Fee: We reserve the right to charge you half of the treatment fee for cancellations less than 24 hours and for late shows greater than 10 minutes. This fee will be charged to the card on file &/or must be collected at the next scheduled appointment.

Full Price Required: If you do not call to cancel an appointment within 24 hours and you "No-Show", you will be charged the full price of the session for which you are booked. This fee will be charged to the card on file &/or must be collected before the next appointment can be scheduled.

Payments, Returns & Refunds:

Payment Policy: All services and products are self-pay. We do not accept insurance for payments. Payment is due in full at time of service and products are rendered or upon other written agreement. We accept cash, check, Visa, MasterCard, Discover, American Express and Gift Cards. No returns No refunds will be issued; ALL PAYMENTS ARE FINAL. Gift certificates will not be replaced if lost, stolen or damaged nor will refunds be administered. Gift certificates are redeemed for their dollar value through our services.

Gift Cards: Our gift cards can be used for treatments or products, you may purchase your gift certificate at our office. There are no refunds or no replacements if lost, stolen or damaged. Gift cards are redeemed for their dollar value through our services.

Product Return: Products may be returned for Reveal Medical Aesthetics, Inc. office credit only and only if returned within 10 days of original purchase and if not opened or used. We may issue a Reveal Medical Aesthetics, Inc. office exchange for unused, pre-paid services. This applies only if the request is made within 3 months of original purchase. No discounts shall be issued on any selected services (list price will be charged) in the exchange and if original services were purchased at a discounted rate, only discounted rate shall apply to the new exchanged services at list price.

Defective Products: If a product should become defective there is a "10 days from purchase" exchange policy. Products will be replaced immediately if in stock otherwise will be ordered. 

Refunds: No returns No refunds will be issued; ALL PAYMENTS ARE FINAL. Any credit on an account can be used towards products or services offered at Reveal Medical Aesthetics. Return Check fee of $50 on all returned check payments

Confidentiality HIPPA: Please do not discuss your medical or otherwise personally identifiable health information in emails others than through your Aesthetics Pro portal. Aesthetics Pro is a HIPPA compliant web portal, please allow us 24 hours to respond to any of your questions or comments.
